At Back in Form we offer bulk billing services to our patients who have a Chronic Disease Management Plan from their doctor for Physiotherapy.
You may be qualified for up to 5 sessions (bulk billed) per calendar year. Please consult your doctor for eligibility.
You will need to bring your Medicare card and the doctor’s referral at the time of your initial consultation.
Medicare benefits are available for certain services provided by eligible allied health professionals to people with chronic conditions and complex care needs who are being managed by a GP or medical practitioner using certain Chronic Disease Management (CDM) Medicare items or are enrolled in a Health Care Home. The allied health services must be recommended in the patient's plan as part of the management of their chronic condition.
The Transport Accident Commission can pay the reasonable costs of physiotherapy services when required as a result of a road accident injury.
The TAC will periodically review an injured worker's entitlement to physiotherapy services to ensure that the treatment and services remain reasonable for the work-related injury and/or illness and are payable under the legislation.

DVA Department of Veteran Affairs White and Gold card programs allow you to receive your physiotherapy under your doctor’s referral without any out-of-pocket expenses. You will need to provide DVA white or gold card details at the time of booking and bring a copy of your doctor’s referral with you to your initial consultation.
The WorkSafe Agent (the Agent) can pay the reasonable costs of physiotherapy services when required as a result of a work-related injury or illness under Victorian workers' compensation legislation (the legislation).

Back in Form Physiotherapy is proudly registered as providers under the National Disability Insurance Scheme (NDIS).
The NDIS funds Allied health and other therapy needed because of a person's disability, including physiotherapy.
